Tapestry Accident & Incident Reporting Procedure

Purpose

To ensure all accidents and incidents are recorded accurately, consistently, and promptly on Tapestry, providing a clear record of what happened and any actions taken.

When to Complete an Accident or Incident Record

An accident or incident record must be completed whenever:

  • A child sustains an injury, however minor.
  • A child is involved in an incident that may cause concern or require monitoring.
  • First aid is administered.
  • There is an accident involving another child, visitor, or member of staff where a record is required.

Procedure

  1. Ensure Immediate Safety
  • Attend to the child and assess the situation.
  • Administer first aid if required.
  • Seek assistance from a qualified first aider when necessary.
  • Inform Management of any significant accidents or incidents.
  1. Record the Accident/Incident on Tapestry

As soon as possible after the event, create an Accident or Incident entry on Tapestry.

  1. Complete All Required Fields

Ensure the following information is entered:

Child's Details

  • Full name of the child.
  • Date and time of the accident or incident.

Description of Accident or Incident

Record:

  • What happened.
  • How it happened.
  • Any obvious cause.
  • Any injury sustained.

Use clear, factual language only. Do not include opinions or assumptions.

Example: "Child tripped whilst running and fell onto the floor, resulting in a small graze to the left knee."

Injury Details

Clearly state:

  • The location of the injury on the body.
  • The type of injury (graze, bump, cut, bruise, etc.).

Treatment Given

Record:

  • First aid administered.
  • By whom.
  • Any monitoring required.
  1. Complete the Additional Information Section

The Additional Information section must include the area where the accident or incident occurred.

Staff must select and record one of the following locations:

  • The Hive
  • The Nest
  • The Garden
  • Toilets
  • Changing Area
  • Cloakroom

Example: "Location: The Garden"

If applicable, include any other relevant information relating to the circumstances surrounding the accident or incident.

  1. Inform Parents/Carers
  • Inform the child's parent/carer at collection or sooner if required.
  • Ask the parent/carer to review and sign the record on Tapestry using the app.
  • Ensure the record is acknowledged before closing the report.
  1. Notify Management

Inform the Nursery Manager or designated person immediately if:

  • The injury is serious.
  • Medical treatment is required.
  • There is a safeguarding concern.
  • Multiple accidents or incidents are occurring in the same area.
  1. Review and Submit

Before submitting:

  • Check all information is accurate.
  • Ensure spelling and grammar are clear.
  • Confirm the accident/incident location has been recorded in the Additional Information section.
  • Verify that treatment and parent notification have been documented.

Staff Reminder

All accident and incident records must be:

  • Accurate
  • Factual
  • Completed promptly
  • Signed by parents/carers
  • Recorded with the correct location:
